[6] Diocese in which his birthplace is located
[7] Capuchin circumscription to which the friar belonged
[8] Date of investiture (day - month - year)
[9] Date of temporary profession (day - month - year)
[10] Date of perpetual profession (day - month - year)

For those who died as deacons:
[11] Date of diaconal ordination (day - month - year)

For those who died as prelates:
[11a] Date of presbyteral ordination (day - month - year)
[11b] Date of episcopal ordination (day - month - year)
[11c] Name of titular church, if applicable

[12] Date of death (day - month - year)

Account of the friar's life

Following the factual data, we ask that you include a brief account of the friar's life, including the major moments of his life: events, assignments, responsibilities, and accomplishments, and a description of the type of person he was, particularly in ways that might encourage other friars. Obituaries should be submitted in the vernacular, although Latin texts are also acceptable. They should be sent to:

Editor, Analecta
Capuchin General Curia
Via Piemonte, 70
00187 Rome, Italy
FAX: (39) 06 - 48-28-267

It is even more convenient for us to receive the death notices (as well as articles) through e-mail: analecta@ofmcap.org

If the deceased friar once served on the staff of a fraternity dependent upon the general minister (general curia, Frascati, International College, etc.), we ask that a photograph of the friar be sent to Analecta with his necrology via regular mail (post).
